Thank you for your interest in Champions Preparatory Academy for your family.
Champions Preparatory Academy welcomes intent to enroll forms. CPA currently serves students in grades Preschool through 3rd grade.
Our Applocation Process- Made Easy!

All current students (grades K-3rd) must complete the “Intent to Enroll” form by March 31st.

New Students- all applications are completed and submitted online-

To enroll for preschool, please call to schedule a discovery visit.

Submit Required Documents
Pay Fees: There is an application fee associated with enrolling in the academy.
Wait for the Admission Decision
Acceptance: If you are accepted, you'll typically receive an acceptance letter or email. This will contain information on how to confirm your enrollment.

Once you receive your application link:

Your family will create an online account, allowing return to the application at any time

An application needs to be submitted for each child

Required supporting documents must bge uploaded during the online process

Please note that submission of an application is not a guarantee of acceptance
PROOF OF RESIDENCE DOCUMENT REQUIREMENTS
The following documents should be submitted with the Enrollment Packet:

ORIGINAL BIRTH CERTIFICATE

IMMUNIZATION CERTIFICATE (OR RELIGIOUS EXEMPTION)

CERTIFICATE OF VISION, HEARING AND DENTAL EXAM

SOCIAL SECURITY CARD (OPTIONAL)

It is not mandatory to provide a social security card/number.

TRANSCRIPTS/REPORT CARD FROM PREVIOUS SCHOOL (Kindergarden and above grades)

DISCIPLINE REPORT

PROOF OF LEGAL GUARDIANSHIP

Note: If legal custody of a child is split between two parents, in addition to the documents listed above, you must also attach a certified copy of the most recent court order identifying each parent’s respective award of physical custody. You are responsible for immediately informing the school of any changes to the court order.
Should a family fail to provide proof of residency or should it be determined that a family has provided false information, the application will be void.
